Job description
At Well Care Health, our Pharmacists are onsite in various hospitals to assist during the discharge process of patients. They perform medication reconciliation, MTM, DURs, education training, prior authorization assistance, and ensure that most patients leave the facilities with their discharge medications, improving compliance and adherence.
The team follows up with the patients within 48 hours, 15 days, and 30 days. These phone calls or visits, enable them to resolve any issues or questions patients may have regarding their medications. The goal of the program is to prevent any readmission within 30 days.
Position available for Clinical Part-time Pharmacist to provide MTM services.
Compensation is on a per patient basis.
Pharmacist must be willing to commute to facilities/hospitals as needed.
