Seeking an ESS Case Manager at Great Hearts America in Phoenix, AZ.



JOB SPECIFICATION


POSITION: ESS Case Manager


DEPARTMENT: GHAESS – AZ - ESS


LOCATION: 101GHA – GH (AZ) Phoenix Home Office


REPORTS TO: National Director of ESS


FLSA STATUS: Exempt, Full-time

At Great Hearts, our Special Education Teachers and Coordinators are integral members of our faculty, participating in all faculty-wide events and PD, and also bringing the specialized expertise that is necessary for the success of all our students. Great Hearts aims to serve all students within our special education population in a manner that affirms and reflects their dignity and rightful participation within the larger student body and academy. Through consultation, resource, inclusion, and co-teaching models, the Special Education team provides essential support, following an Individualized Education Plan, to ensure each student is served in the least restrictive environment possible.


Great Hearts ESS Case Manager Overview:



Position Summary


The responsibilities of the Great Hearts Home Office ESS Case Manager are both ongoing and intermittent.


The core responsibility of the Specialist is to be of service to academies by providing case management support to academies that have vacancies in ESS due to the hiring shortage or are shorthanded due to an unforeseen resignation.


Intermittently and especially during long breaks in school operation (e.g. the summer), the ESS Case Manager may also assist in further developing the systems used by the Great Hearts ESS program to manage special education cases, draft legal documents, and maintain compliance with state and federal law. Examples of this include developing process documents, revising templates and document software, etc.



Duties and Responsibilities


1.Manage in entirety an agreed-upon selection of student cases. To wit:


a.Track timelines for IEP and MET meetings


b.Collaborate with academy staff to set up IEP/MET meetings and send out Meeting Notices


c.Collaborate with academy staff, related service providers, and parents to collect student information to prepare IEP/MET documents


d.Prepare IEP/MET Documents


e.Lead IEP/MET meetings


f.Finalize compliant IEP/MET documents, write and distribute PWNs within expected timelines


g.Collaborate with academy staff, related service providers on progress reports


h.Write and distribute progress reports to parents.


i.Update caseload documents, and maintain SPED programs in Student Information System (PowerSchool)


2.Coaching/consultation on implementation of IEPs


3.Regular Academy Visits. Purpose(s):


a.Connect with academy faculty & leadership to build relationships and trust


b.Follow-up on/collect needed information and/or set meetings


c.Connect with ESS team to solve tactical issues and make strategic plans


d.Coach/consult with GE teachers regarding IEPs


4.Provide support in transition of cases back to academy staff


5.Intermittently: assist in further developing the systems used by the Great Hearts ESS program to manage special education cases, draft legal documents, and maintain compliance with state and federal law.

Education/Certification Requirements


Bachelor’s degree required


Master’s degree and certification in special education field preferred



Additional Requirements


Type of fingerprint clearance: Must possess or obtain valid Arizona Department of Public Safety fingerprint clearance card.


Physical functions: Requires but is not limited to standing, sitting, repetitious hand and body movements, bending, squatting, and reaching. May be required to move or position equipment up to 15 pounds approximately.
