Job description
The Transitions Program is part of a grant funding services supporting persons in our county jails within our catchment area (Taylor, Callahan, Jones, Stephens and Shackelford Counties)
The duties of the Transition Case Manager will include consulting with jail staff on mental health screenings when persons with mental health issues/history are arrested and identified. This person will also assist jail staff in developing release plans to transition to outpatient and reentry services successfully.
This position will be responsible for providing case management, skills training, psychosocial rehab and crisis services to the Transitions Caseload with the goal of maintaining their mental stability and
to reduce recidivism back into the county jail(s).
This position provides a variable level of support to consumers with mental health issues in an integrated Psychosocial Rehab treatment team structure. This includes, but is not limited to scheduling, coordinating and facilitating housing opportunities for consumers through housing development activities; helping consumers secure and maintain affordable housing; and provide housing advocacy and other supportive services to consumers.
This position requires the ability to: identify problem/need areas; work under pressure and stressful situations in a professional manner; be thorough in assessment and documentation; locate and utilize a variety of community resources to meet the needs of each individual; assist in negotiating and advocating for each individual with service entities outside the Center; and develop a positive relationship with each consumer. This position provides supported housing services for qualifying individuals
This position will be directly supervised by the Admissions/Crisis/TCOOMMI Program Administrator.

-Requires a Bachelors Degree in Psychology, Human Services, Criminal Justice or related field from an accredited college or university (social work degree preferred).
-Must be eligible to be certified as a Qualified Mental Health Professional
-Prefer previous mental health and/or criminal justice work experience with adult consumers
-Must possess a valid Texas Driver's License and be an insurable driver.
