Responsibilities:
• Perform clinical assessment and identify patient needs.
• Document, prepare, and complete patient Plan of Care. Track progress and revise as needed.
• Meet mandatory continuing education requirements.
• Plan and provide appropriate treatment for neuro-muscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
• Assist and instruct patients and families in transfer techniques appropriate to patient abilities. Instruct patients and families on the use of equipment, such as walkers, wheelchairs, canes, etc., in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs.
• Promote company mission and administrative policies to ensure the quality of care.
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program
• Current Texas State License as a Physical Therapist
• Current Texas Driver’s License and reliable transportation
• Six months of experience as a Physical Therapist in an acute care or rehabilitation setting. Pediatric experience is preferred.
• Effective interpersonal relations and communication skills
• Knowledge of durable medical equipment
• Ability to work a flexible schedule and travel locally
• Able to work in a patients home in various conditions
• Excellent organizational and time management skills
• Proficient in Microsoft Office
• Able to maintain a high level of confidentiality
• Proof of current CPR certification
• Ability to do heavy lifting, bending, pulling, pushing, and standing. Prolonged standing and walking are required.
