As a Hospice Care Registered Nurse (RN) at Cornerstone VNA, you'll have the freedom to provide care to patients in the comfort of their own homes. Independence is key, but don't worry, support is just a phone call away. Your manager and team are always there to assist you whenever needed.
During each visit, you'll coordinate care delivery, ensuring that your technical nursing skills are top-notch and your compassionate approach to end-of-life care shines through. Adjusting the care plan when necessary, you'll collaborate with the patient's physician and the Hospice Care Team.

To excel in this role as a Hospice Care Registered Nurse (RN) at Cornerstone VNA, a current state of New Hampshire RN License or compact state RN license is essential. As you'll be visiting patients in their homes, having a Valid driver's license in good standing and vehicle insurance is necessary. In addition to the required licenses and documentation, you'll need excellent communication and interpersonal skills to effectively collaborate with patients, their families, and the Hospice Care Team. Compassion, empathy, and a genuine desire to provide the best possible care are vital traits.
Being a problem solver and a quick thinker is important when faced with unexpected situations or changes in a patient's condition. Your ability to adapt to various healthcare settings and handle complex medical situations will contribute to your success in this role. Lastly, strong organization and time management skills are crucial to coordinate care effectively and ensure that each patient's needs are met.
