Role Summary:  
As a Physical Therapist, you'll be an essential member of our healthcare team, providing Physical Therapy services in compliance with all applicable laws and standards. You'll work under the direction of a physician and nursing supervisor to deliver skilled Physical Therapy care to our clients in their homes.

Key Responsibilities:

• Conduct initial and ongoing assessments, including OASIS assessments.
• Develop and revise treatment plans with physician approval as necessary.
• Participate in the development and implementation of interdisciplinary client care plans.
• Instruct and counsel clients and families on physical therapy programs.
• Supervise Physical Therapist Assistants in accordance with regulations.
• Communicate with physicians and team members regarding client status and care.
• Document clinical/progress notes promptly and accurately.

Qualifications:

• Graduate from an accredited Physical Therapy program.
• Valid Florida PT license.
• Current BLS Certification.
• Valid driver's license with a reliable and insured vehicle.
• Preferably one year of experience as a Physical Therapist, with home healthcare experience, is beneficial.
• Proficiency in electronic medical records is preferred.
• Ability to work independently with excellent observation, judgment, and communication skills.
