TITLE OF IMMEDIATE SUPERVISOR: CLINICAL DIRECTOR

RISK OF EXPOSURE TO BLOODBORNE PATHOGENS – HIGH

DUTIES

Provide skilled nursing services, on an intermittent basis, to patients, in their homes.

RESPONSIBILITIES

Assist in the development, and implementation, of the nursing care plan.

Evaluate the effectiveness of nursing intervention and updates the patient care plan, as needed.

Communicate with the physician, at least, every two weeks, regarding patient's status.

Communicate with other team members, in order to provide comprehensive patient care.

Ensure that all OSHA/ACHC standards are in compliance.

Participate in committee activities and meetings.

Discuss, with the Clinical Director, on a weekly basis, the patient’s condition and required care and frequency of visits to be scheduled, for the following week.

Assume responsibility for self-development by continually striving to improve his/her nursing knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings, and individual research and reading.

Observe confidentiality, and safeguards, on all patient-related information and intradepartmental data.

Perform other duties, as assigned, by the Clinical Director.

COORDINATES THE ADMISSION OF THE PATIENT TO THE AGENCY

Conduct initial, and ongoing, comprehensive assessments of the patient’s needs, including, Outcome and Assessment Information Set (OASIS) assessments, both initially, and as required.

Work with Discharge Planners to identify, and plan, home infusion therapy referrals.

Assess the patient on the initial visit, including an assessment of home safety, adequacy of refrigeration, storage, and cleanliness.

Initiate the patient plan of care and make necessary ongoing revisions; provide service, in accordance with reimbursement, ethical, and legal guidelines.

Initiate appropriate preventive nursing procedures.

Administer venipuncture and handle insertion of intravenous needles and catheters.

Prepare infusion solutions, with the addition of medications.

Initiate, monitor, and terminate infusion solutions, and additives, and establish flow rates (if not done by the physician).

Prepare clinical, and progress, reports and make referrals for other services, as appropriate.

Inform the physician, and other appropriate personnel, of changes in the patient's needs.

Train, both, the patient and caregiver and assess the ability of patient, or caregiver, to manage when staff is not present.

Maintain, and submit, written clinical and Agency records, as deemed necessary, to include: an initial assessment, a care plan, daily notes, and weekly schedules, summaries of service, change orders, re-certification, and discharges.

PROVIDES SKILLED NURSING CARE AS OUTLINED IN THE PATIENT CARE PLAN

Nursing services, treatments, and preventive procedures, ordered by the patient's physician, that require substantial specialized skill.

Initiate preventive, and rehabilitative, nursing procedures, as appropriate for the patient's care and safety.

Observe signs and symptoms, and report to the physician, the patient’s reactions to treatment, including changes in the patient's physical or emotional condition.

Teach, supervise, and counsel the patient, and family members, regarding the nursing care needs and other related problems of the patient, at home.

SUPERVISES HOME HEALTH AIDES

Supervise, and evaluate, the care given by the Home Health Aide, every fourteen (14) days, and more frequently, if necessary, and submit completed written reports.

Participate in periodic conferences, with the Home Health Aide Coordinator, concerning the Home Health Aide's performance.

JOB CONDITIONS

Travel is required, by car or airplane to local, out-of-town, or state seminars, conferences, or meetings.

The ability to communicate well, both, verbally, and in writing, in English is required.

The ability to access patients’ homes, which may not be routinely wheelchair accessible, is required.

Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.

Physical activities include, walking, sitting, stooping, standing, lifting, and turning of patients.

EQUIPMENT OPERATION

Use of multi-line telephone, calculator, and medical nursing equipment, i.e., glucometer, etc.

COMPANY INFORMATION

Has access to all patient medical records, and patient financial accounts, which may be discussed with Clinical Director, the Director, the Executive Director, the Director of Reimbursement, and the Quality Assurance Department.

QUALIFICATIONS

1. Must be a graduate of an accredited School of Nursing.

2. A BSN is preferred.

3. Must be a Registered Nurse licensed by the state.

4. Must have a minimum of 1-year experience, as a professional nurse, in an acute care setting.

5. Must have a criminal background check.
