Job Role: Lead Practitioner-adults in Barnet
Responsibilities will include: • The mentoring and supervision to staff within the team ensuring professional standards and best practice. • Ensuring that the responsibility of safeguarding adults at risk is a fundamental element of this job and that the principles are embedded in all procedures, practices, professional advice, decisions and support. • Working alongside and when required deputising for the team manager to prompt an environment of a supportive team enabling them and yourself to work with individuals to promote and support independence choice and control with wellbeing at the heart of everything we do. • Conducting individual complex assessments and safeguarding enquiries to establish clear, relevant, proportionate, and appropriate options for the individual to meet their identified outcomes. • Working in partnership with adults, their families, carers and advocates,
colleagues, external organisations to provide appropriate, professional social work support for vulnerable older people, adults with physical and sensory impairment or learning disability and carers We offer a 36 hour a week, hybrid working with a mixture of office and home working, although staff are welcome to be office based fulltime if they wish. As a Social worker you must have a degree in Social work with a minimum 2 years post qualifying experience and passed your AYSE, experience in an NHS Establishment or a local authority is desirable.
To act in accordance with SWE registration requirements and Code of Conduct for exercising accountability and professional practice.
To provide professional Social work assessments to support the commissioning process in line with care management and continuing care.

Employers will be keen to see that you have a friendly and caring attitude, and understand and respect people who need healthcare support.

Previous experience and/or qualifications in Social work are essential to apply for this role.
