Location: Brentwood
Banding/Grade: Band 6/7
Pay rate (Assignment rate): £27 - £31 per hour
Contract Length: 3-Months
Shift Pattern: Monday - Sunday, 08:00 am - 20:00 pm

Duties:
- Cover a Frailty Virtual Ward/IRS Nurse Specialist caseload
- Working virtually or working face to face in community in Thurrock, Basildon, and Brentwood localities
- Complete Initial assessments, make medication changes, reviews of patients care plans
- Complete FrEDA template, on assessment and CFS
- Provide education to patients, carers, and the wider MDT. To keep accurate, detailed up to date records of patients
Requirements for theFrailty Nurse Specialist role:
- Degree or equivalent in Nursing (RGN qualified) with active NMC Registration (Adult)
- Understand and have awareness of NICE recommendations around the management and assessment of Frailty and Elderly patients
- Registered Band 7 Epilepsy Clinical Nurse Specialist
- Recent or present knowledge and working experience of caring for those with epilepsy and other related neurological conditions including undertaking annual risk assessment for sodium valproate
