Job description

Job Description / Role: Support the role of PCN clinical pharmacists in the delivery of clinical pharmacy services to people in their own homes or local care homes Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing Improve the use of medicines and help patients with medication concordance Assist with medication safety monitoring systems including uncomplicated medication reviews Liaise with hospital teams, local community pharmacies, clinicians and administrative staff to resolve medicine-related queries. Monitor practice prescribing and adherence to practice, local and national prescribing policies Improve service and quality through audit and searches Help identify patients at risk of medicines use issues, including waste, safety and over-use Help manage simple medication requests through liaison with the clinical pharmacist team where appropriate Help to maintain and achieve Quality Outcome Framework targets for both practices Personal and Professional Development: The post holder must take responsibility for their development and learning by: Committing to or currently undertaking the CPPE Primary Care Education Pathway Meeting GPhC CPD requirements for registration on an annual basis Completing mandatory training required by the PCN Requirements of Role The post holder Must: Ensure principles of openness, transparency and candor are observed and upheld Have an understanding of GDPR and patient confidentiality Undergo safeguarding training and adhere to practice safeguarding policies
