Job description
The Rotherham CAMHS Service have an exciting opportunity for a Band 6 CAMHS Transitions Practitioner to join our Getting Help Pathway Team. The successful candidate will join a multi-disciplinary team completing assessments and providing evidence based interventions, advice and support to professionals, parents and young people, working jointly across CAMHS and adult mental health services.
We are looking for a CAMHS transitions Practitioner who is passionate about delivering high quality and effective services to young people and their families focussing on transitions from CAMHS to adult services. You will be young person centred in your approach and have excellent clinical skills, experience of multi-agency working and be able to demonstrate the ability to work independently and autonomously. You will have experience of working within a children and young people safeguarding framework.
