Education Health and Care Plan Coordinator (Contract/Temporary)
The Education Health and Care Team is responsible for performing statutory duties related to special educational needs and disability (SEND) assessments and placements. The team is seeking to hire a fulltime Education Health and Care Plan Assessment Coordinator (Band H) to work within their ranks. The successful candidate will be responsible for coordinating statutory assessments maintaining Education Health and Care (EHC) plans for children and young people aged 025 providing planning and decisionmaking support for educational placements and SEN provision including personal budgets as outlined in EHC Plans monitoring and reviewing education placements and provision for children and young people with EHC plans in Rotherham and ensuring that annual reviews are conducted and that educational settings receive relevant advice and guidance to fulfill their statutory duties.
- Ensure effective coordination of information and advice regarding the statutory assessment of children and young people with special educational needs and disabilities while collaborating with other practitioners to create and review highquality Education Health and Care Plans (EHC).
- Apply the principles outlined in the SEND Code of Practice (2015) and the Children and Families Act (2014) while working closely with parents children and young people to create EHC Plans that incorporate their views and wishes.
- Provide advice and support to schools colleges other professionals and families on matters related to the assessment planning and review process.
- Promote strong interagency cooperation and ensure all parties contribute to the assessment process.
- Draft highquality EHC Plans in accordance with statutory procedures and discuss them with the family to ensure clarity accessibility and outcomefocused planning.
- Collaborate with families to agree on the final version of the EHC Plan clarifying the roles of all parties involved in implementation including identifying appropriate school provision and supporting and challenging Education Health and Care Providers to meet the plans requirements.
- Monitor statutory annual reviews including key phase transitions and make necessary amendments to the EHC Plan while adhering to statutory timeframes.
- Organise and chair annual reviews of EHC Plans for young people without educational placements.
Requirements
1. Proficient understanding of various special educational needs and disabilities.
2. Indepth understanding of pertinent legislation including the SEND Code of Practice (2015) Children and Families Act (2014) and safeguarding procedures.
3. Ability to resolve conflicts and foster constructive relationships with stakeholders.
4. Comprehensive knowledge of the services and agencies involved in Education Health and Care (EHC) assessments and plan implementation.
5. Strong written and verbal communication skills suitable for diverse audiences.
6. Experience in utilising various computer applications and ICT tools such as word processing spreadsheets databases presentation software the Internet and email.
7. Recent and relevant experience working in a role or environment directly related to special educational needs and disabilities.
Required to hold a full driving licence
