Job description

The Senior Programme Manager will work within Humber and North Yorkshire ICB and to the broader partnership of the York Health and Care Partnership to support the progress of the partnership priority projects, and the adoption and spread of Population Health Management Tools and Techniques across the system. The role involves working strategically to support the new Partnership Board and Leadership team in delivering a number of set priorities as part of its emerging role as the Place Based Partnership for York within the North Yorkshire and Humber Integrated Care System. These priorities are being built around population health data, but this role exists to turn this insight and partner priorities into reality, 'doing' real projects and providing effective advice and hands on support to local teams and partners. It is a leadership role, tasked with ensuring integrative approaches between health partners and between health and social care are taken, to improve quality of care, empower clinicians, engage seldom-heard communities and deliver population health management in our place. The role supports two key programmes of work: Programme #1 York Health and Care Partnership Our York Health and Care Partnership is an exemplar of the way place-based partnerships under a larger Integrated Care System are starting to function. Drawing from a cross-section of leaders in health and care, we are asking people work together in a new way, shaping services and a new culture within York which will lead to integrated services and more joined-up care. This role will offer senior programme management resource for the York Partnership, working closely with CCG and council executives, our facilitator and other partners. Programme #2 York Population Health Hub Under the recently established Population Health Hub for York, a virtual team of data analysts, service improvement managers and public health specialists, are working together to embed a population health approach and population health management techniques into our work as a health and care system. The work centres around three pillars: Enabling: training, developing capacity and supporting the York health and care system to use population health data, and PHM as a tool Analysing: delivering the JSNA, making it useful and used Doing: tangible, real work leading PHM projects which show the benefit of this approach The post holder will offer senior programme management resource for the Hub, and provide the capacity we need to deliver its objectives, whilst the Director of Public Health for City of York Council provides specialist and technical population health expertise into this work and is the Senior Responsible Officer for the project. As a Senior Manager, you will also be required to work closely with colleagues at Place and System Level in supporting the delivery of operational and strategic goals. You will be expected to act as a team-player and support your colleagues, to develop and progress against the teams deliverables. Specific requirements of the role include: Working effectively as part of a wider team to deliver a successful Place-Based Partnership in York and support a range of products and supporting materials to help local partners Influencing without formal authority, and taking an autonomous approach to work Cajoling senior leads across the system to drive forward progress on population health management Understanding of Population Health and prevention priorities, and comfortable with discussing and presenting highly complex data and health intelligence Engaging, listening, leaning and co-designing the Alliance and population hub with partners Dealing well with uncertainty, on new projects with significant possibility of changing goalposts and new tasks 'thrown at you' but with real scope for finding yourself at the heart of an innovative 'place-based' commissioning model. Work closely with the AD for System Planning and Improvement to co-ordinate an approach to planning that clearly links the priorities of York Health and Care Partnership with NHS operational planning, working with place and system colleagues to ensure all partner organisations are engaged in development of the partnership and place plans. A key expectation of the role is that you will be able to operate as a network with your colleagues who represent other Places across the system. In the role of Senior Manager you will provide senior input to the delivery of the team objectives and manage and line manage support team members to ensure an effective support offer to systems. Key Relationships The post holder will have key relationship with the following teams, departments and organisations covering all health and care organisations across York place: Partnership work programme and project leads/senior responsible owners and members of the boards or groups Programme managers from all partnership work programmes York Health and Care Partnership Members of staff from partner organisations including Chief Officers, Chief Executives and other senior leaders Humber and North Yorkshire ICS DHSC Office for Health Improvement and Disparities Improving quality and outcomes To develop and manage effective relationships with key senior stakeholders, both internal and external to the ICS To work with partners to ensure that our operational delivery teams have access to the solutions it needs to effectively manage population health. To work with the system partners to monitor infrastructure and resource (including our workforce) requirements to ensure we are capable of delivering effective population health management Enabling patient and public involvement To ensure all public and patient contact across the Population Health Management programme is of the highest professional standard To embed patient and public involvement within Humber and North Yorkshire at all levels of decision making, requiring frequent interactions with patients and the public. Promoting equality and reducing inequalities To uphold organisational policies and principles on the promotion of equality To create an inclusive working environment where diversity is valued, everyone can contribute, and everyday action ensure we meet our duty to uphold and promote equality Partnership and cross boundary working To work across our system to promote understanding of the role of population health support services in solving the challenges the health system faces To work across the system to ensure that services are effectively aligned to future requirements To collaborate and work in partnership with local and system leaders across the wider system to deliver successful outcomes To work flexibly across the group, providing support and additional resource to workstreams and senior sponsors where required Operational requirements To support the successful adoption of widespread population health management tools and techniques, aligned to the systems strategic objectives and policy driven expectations. Provide overall management to the function of the Population Health Management Team Develop and implement qualitative and quantitative measures to determine performance against the strategic objectives Report progress against the strategy through personal representation at senior management forums and by written reports. Tailoring delivery to meet the needs of the audience Ensure that best practice is developed and delivered at system, organisational and departmental levels. Challenge ways of working and persuade, motivate and influence other senior managers to realign their practice where necessary. Ensure that appropriate strategies are employed in line with business objectives and that these are fully cascaded. Personally lead, support and contribute to formal negotiations with senior level internal and external stakeholders, providing a high level of negotiating expertise to secure the most advantageous arrangements. Advise on innovative opportunities and support all departments in their strategies and programmes to maximise service benefits. Oversee the tracking of progress against plans and transition milestones, ensuring appropriate processes are in place to flag issues, risks and concerns with the relevant stakeholders. Ensure the securing of value for money, assuring relevant factors such as quality and governance. To monitor, interpret and quality assure progress against deliverables. To develop business plans and provide expert strategic advice and guidance Devise, manage and update policies and procedures, ensuring the adoption of best practice methodology, rules, standards and thresholds Contributes to the development and implementation of general policy and service development. Develops and implements strategies for improving performance and processes, ensuring stakeholders representation. Drives and leads development and improvement of processes. Drives process efficiency in the continuous development of the end-to-end cycle and its associated performance metrics. Accountable for short, medium and long-term business plans, achieving quality outcomes. Responsible for linking people, resource and strategy to organisational strategy. Act as senior lead working internally and externally to deliver projects, initiatives and services to time and in a cost - effective way. Operate in a highly political and sensitive environment. Support the portfolio of programmes/projects to demonstrate value for money tracking, managing and delivering agreed benefits. Provide advice and prepare strategic reports and briefings for senior stakeholders (including directors) In depth analysis, interpretation and production of highly complex and multiple reports.
