More detail about the role: We have the exciting opportunity to appoint an Assistant Practitioner, Community Therapy to join our enthusiastic Community Therapy team based in South Molton. We are looking for a practical, proactive and enthusiastic individual with good communication skills. We are looking for an applicant with experience of working in a Care or Rehabilitation setting with the passion to work in the Community Therapy team based at South Molton Community Hospital. The post holder will work in the Community with patients registered under South Molton and Wallingbrook (Chulmleigh and Winkleigh) medical centres and cover a rural area close to Exmoor in beautiful North Devon. You will be working with people with a wide range of disabilities and acute or long-term conditions. The role of the Assistant Practitioner is to complete holistic assessments with patients in their home environments. The role holder develops, delivers, monitors and evaluates the outcome of a therapy plan based on the assessment. The role holder manages their own caseload and may delegate to and support B3 clinical support workers. The role holder may deliver 1:1 support and/or organise, run and deliver group sessions including strength and balance classes. The role addresses health promotion, rehabilitation, equipment prescription and urgent, unplanned work to prevent Hospital admission and, or, support discharge. The role includes work in patients homes, residential and nursing homes and remote/virtual working. The post holder will use electronic healthcare systems EPIC as well as paper based documentation. They will be an integral part of the team in terms of service delivery as well as service monitoring and development. The Community Therapy team has twelve members including Occupational Therapists, Physiotherapists, an Occupational Therapy Apprentice, Assistant Practitioners and Clinical Support Workers. We are a supportive team and the role holder will have personal and professional development opportunities. Our integrated Health and Social Care team includes the Community Therapy team, Community Nursing, Community Matron, Rehabilitation Nurse and Adult Social Care team. We work closely with social prescribers in the GP practices for links with and inclusion of the people we work with into community activities. Typically, the day starts with members of the team triaging new referrals and a team meeting to prioritise and plan the day. We positively encourage joint working between members of the community therapy and broader health and social care team.
