Lancashire & South Cumbria Trust Pennine locality has an exciting opportunity for an Occupational Therapist to join our community rehabilitation team.
The team includes Falls Clinical Specialist, Occupational Therapists, Assistant Practitioners, Technical Instructors and Community Rehabilitation Assistants. We work with the wider multidisciplinary team and support long term rehabilitation with patients in their own home’s. We attend Integrated Neighbourhood Team meetings weekly to support the health and well-being of Blackburn with Darwen residents using a multi-disciplinary approach with health, social care and voluntary sector partners. We work closely with our Intensive Home Support Service colleagues to support a patient remain at home.
We are looking to appoint an enthusiastic Occupational Therapist to join our service. The role will involve assessing patients, planning treatment and coordinating support and equipment to meet the patient’s needs under the supervision of senior Occupational Therapists within the team. You should have the ability to travel within the Blackburn with Darwen area and be able to manage and prioritise your own workload.
The successful candidate will need excellent communication skills who is confident to work autonomously as well as part of a team.
Work is ongoing to transform adult physical health services across the Pennine Lancashire Footprint supported by commissioners within the Lancashire and South Cumbria system. This transformation aims to ensure service users in Blackburn with Darwen and East Lancashire receive equitable service provision from one service provider. From around Summer 2024, the service provider will be East Lancashire Hospitals Trust. Staff employed by LSCFT will therefore transfer from LSCFT to East Lancashire Hospitals Trust as part of this wider transformation plan.
This post is subject to an enhanced DBS check and will require the successful candidate to be a car driver for this community role.
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
