Remuneration: Circa £120,000+ pro rata

Description: Part-time Dentist required for 2 days  ( Thurs & Fri ) in an INDEPENDENTLY OWNED well-established practice near Dewsbury, West Yorkshire.
•    £12.50+ PER UDA
•    EXCELLENT PRIVATE POTENTIAL
•    50% split on private
•    Practice offers Invisalign, Facial Aesthetics, Perio, etc.
•    Large 8 surgery practice
•    6 Associates and 3 Hygienist & Therapists for a strong support network
•    Fully computerised surgeries
•    Trios Scanner
•    OPG
•    EMS Air Flow
•    Digital x-rays
•    Rotary endo
•    Full support team including a hygienist/therapist
•    Existing, well maintained list plus new patients
•    NHS access contract (Associate paid per session)
•    BDA Good Award practice
•    Immediate start date, negotiable

- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
