What will I be doing?
- Community Engagement: Work within the community to facilitate effective and timely discharges from assessment bed units, providing essential support to individuals in their homes across the county.
- Holistic Care: Assess and address physiotherapy needs, coordinate equipment ordering, and develop patient-centred goal plans to promote independence and enhance overall well-being.
- Critical Role: Play a vital role in bed management and coordination, ensuring efficient utilisation of resources and contributing to the smooth operation of health care services.
Requirements
- Qualifications: Hold a Diploma or Degree in Occupational Therapy or equivalent, registered with SWE and RCOT, with at least 18 months of experience as a registered Occupational Therapist or equivalent competency.
- Driving License: Possess a clear and valid driving license, as the role requires county-wide travel to fulfil your responsibilities effectively.
- Personal Attributes: Demonstrate empathy, dedication to encouraging independence, effective communication skills, integrity, honesty, and impartiality in your interactions with clients and colleagues alike.
- Specialist Skills: Ideally, have specialist training or equivalent competency in environmental adaptations or relevant experience to excel in this dynamic role.
