Salary: £33,945 p.a. plus benefits
37 hours per week

We have an exciting opportunity for an Assistant Care Manager to join the Discharge Hub within Adult Care as we have restructured all of the Adult Care Service areas to ensure that we can deliver the best services to residents of Swindon.

Your next role

As an Assistant Care Manager, you will work as part of a team supporting residents of Swindon, demonstrating good communication skills and an enjoyment of working in a fast pace environment.

You will join a friendly, supportive and dynamic team where we are committed to providing good quality supervision and continuing professional development opportunities.

We are interested in applicants who have experience of and a passion to work with adults and are committed to providing a person centred and strengths based approach to their work.

The role entails working in partnership with adults and carers to assess need, set up outcome focussed support plans as appropriate as per the requirements of the Care Act 2014 in a multi-disciplinary team you will be a part of how we are re-shaping Adult Social Care.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act for the effective performance of a customer-facing role)

You must hold an NVQ in Care Level 3 or equivalent.
