Job description


An exciting and challenging opportunity has become available for a Band 6 Nurse Assessor. The post holder will see a template of patients clinically, determining fitness for surgery and suitability.

You will work alongside Triage practitioners and independently make decisions and communicating the patient’s fitness for surgery to the wider MDT.

We are a busy department assessing all patients undergoing surgery. You will work closely with a wide MDT, including other hospitals and GP services to ensure a smooth, safe and positive journey for all our patients. We also support weekly Anaesthetic clinics, for complex patient assessment and planning.

Please note you must have your clinical examination skills to apply for this role

You will be able to provide initial direct patient care. You will utilize your Clinical examination skills course by assessing and determining fitness for surgery. You will work within NICE guidance, BADS guidance and the “Basket of 25 “to see your patients.

You will be expected to manage your own caseload of patients and also using a “ buddy “ system to ensure a cohort of patients daily are fully optimized Pre Operatively whilst high standards of quality of care are delivered and maintained.

You will provide first contact assessment, confirm diagnosis and reason for surgery and comprehensively assess patient with minor conditions and minimal co-morbidities. You will be expected to escalate and discuss your clinical findings to determine need for further assessment.

You will work autonomously and be a team player who works flexibly in order to deliver the highest quality care to our patients. You will be enthusiastic, innovative with excellent communications skills along with a willingness to contribute to the improvement and development of services within pre op.

KEY DUTIES

To be able to provide care and emotional support to patients and their relatives who are planned admissions for surgical procedures.

To manage a template of patient caseload in the holistic pre-operative assessment setting, and to recognize and organise individual patient care needs, and to be able to manage own workload and prioritise care.

To ensure the admission process of the patients, including clinical examination, obtaining results of investigations and any action required is comprehensive, concise and valid and the planned admission for surgery is within set protocols.

To communicate clearly, effectively and politely with all levels of the multi-disciplinary team, patients, relatives and other departments.

To undertake in Clinical audit

To oversee Triage Practitioners caseload and assist with any training needs

To work alongside Clinical Nurse Practitioners in the Formal setting

To commence and participate in research/special projects as required/directed.

To contribute to the setting, maintaining and monitoring of pre-operative assessment modernisation within the unit.

To have responsibility for the security and maintenance of equipment belonging to the unit.

CLINICAL & PROFESSIONAL RESPONSIBILITIES

To report, investigate and record any untoward incidences/complaints.

To display a consistently professional manner, and to be always caring and approachable.

ADMINISTRATIVE RESPONSIBILITIES

To ensure the admission process of the patients, including clinical examination, obtaining results of investigations and any action required is comprehensive, concise and valid and the planned admission for surgery is within set protocols.

TEACHING & TRAINING RESPONSBILITIES

To participate in the teaching of student nurses and the in-service training of qualified nurses and health care assistants, and to participate in the induction of new starters.
