3 months Fixed Term
A unique opportunity has arisen within Lancashire and South Cumbria NHS Foundation Trust’s Pennine Network to work as a Band 3 Pulmonary Rehabilitation (PR) Link Support Worker. Pulmonary Rehabilitation Services supports the long term management of respiratory conditions in the community giving patients understanding and autonomy of how to manage their condition in their daily lives. We particularly work with respiratory conditions such as, COPD, Asthma, Emphysema etc. however many of our patients have a multiple health conditions impacting their health and well-being.
We are looking to appoint a dynamic enthusiastic person to join our service.
The role will involve working alongside the Occupational Therapists, Physiotherapist, Nurse, Assistant Practitioner and Community Rehabilitation Health Care Assistant in a multidisciplinary environment.
This will necessitate the provision of a high quality interpretation service wherever required in the Blackburn with Darwen area, including community clinics or in the patient’s homes.
We are keen on recruiting an individual with excellent communication skills. A bilingual speaker wold be advantageous. Good understanding of rehabilitation and have the ability to work autonomously as well as part of a team.

To assist with the delivery of the education programme including interpretation during the sessions. To raise the awareness about respiratory conditions within the ethnic minority communities, and initiate and maintain community development work on behalf of the Pulmonary Rehabilitation Service.
To assist the multidisciplinary Pulmonary Rehabilitation (PR) team in the provision of culturally sensitive services for people with respiratory condition of British Minority Ethnic Groups to access the service and improve completion rates of the Pulmonary Rehabilitation Programme.
