Remuneration: Circa £110,00+

Description: 

Full or Part-time Performer Dentist required for 2-5 days per week (FLEXIBLE) in a busy, mixed, well-established INDEPENDENTLY OWNED practice in West Bowling, Bradford.
•    £13.50+ per UDA
•    50% split on private and labs
•    Invisalign provider advantageous but not essential
•    3 surgery practice
•    2 Associates on-site
•    Fully computerised surgeries - SOE
•    Digital x-rays
•    Rotary endo
•    Existing, well maintained list
•    Full support team
•    Free on-street parking
•    30 mins to Leeds
•    Available now, start date negotiable

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
