Remuneration: Circa £120,000+

Description: Full OR Part-time Performer Dentist required for 2-5 days per week in a busy, mixed, well established, INDEPENDENTLY OWNED practice near Chippenham, Wiltshire. 
•    £5-6K GROSS PER MONTH - PRIVATE EARNINGS
•    5000-6000 UDAs available at £12 per UDA
•    ORTHO, ENDO AND IMPLANT PROVIDER
•    50% split on private
•    50% split on lab bills
•    TRAINING PRACTICE - NEWLY QUALIFIED DENTISTS WELCOME TO APPLY
•    Modern, fully computerised practice
•    iTero Scanner, CEPH scanner, Digital x-rays, Rotary endo, OPG
•    Excellent support team including a hygienist on-site
•    On-site parking available
•    30 minutes from Bath, 35 minutes from Bristol and 30 minutes from Swindon
•    Available now, start date negotiable

- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate

- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
