Job description

KEY RESPONSIBILITIES:

In relation to the support plan designed for the tenant.

 Encourage and facilitate tenant autonomy, personal choice and independence.

 Implement skill training, behavioral management and daily care and support services in accordance with the support plan, reinforcing appropriate tenant behavior utilizing direct feedback, prompts, role-play and modeling.

 Assist in the planning of recreational/social/vocational/educational events and activities, escorting and supporting the tenant as necessary during such activities.

 Ensure effective and regular communication takes place between you and fellow staff team members and team leader, and tenant status is documented in accordance with Elegant Care Services (ECS) procedures.

 To develop and maintain good, professional working relationships with other professionals, outside community agencies and non-service colleagues within Elegant Care Services (ECS).

 Attends training sessions and other meetings as requested.

 Maintains accurate records of all financial transactions utilizing forms approved by Elegant Care Services (ECS).

 Maintains daily accurate records (daily report, activities group, handovers, health logs, incident, accident, health & safety, maintenance repairs logs, loose tablets log and Temperature log)

 To assist and support the Team Leaders in developing and maintaining effective working relationships through positive communication systems, ensuring motivation and promoting good practice in all areas.

 To ensure that the company stated philosophy and ethics of excellence are implemented in practice within the service.

 To ensure a consistency of approach in terms of support is afforded by other staff and outside agencies.

 To ensure confidentiality is maintained regarding tenants.

 To ensure tenants are afforded dignity and respect at all times.

 To ensure that individuals have the opportunity and informed choice to follow any religious or cultural beliefs with or without support.

 Cleaning duties associated to the service

 Abide to policies and procedures of the company

 Complied and attend staff meetings and training.

 Provide support, advocacy and information to the individuals we support.

 Provide emotional and practical support to service users.

 Help service users meet their personal care needs sensitively and appropriately to a high standard, including personal hygiene, assistance with laundry, and preparing meals and drinks.
 Supporting service users to take any prescribed medicine, complying with all medicine policies, including storage, administering and record keeping.

 Assist in devising, implementing and reviewing care plans.

 Provide a safe, comfortable and supportive home for the individuals we support.

 Support service users to actively contribute to the running of their own house and inclusion empowerment.

 Participate in rotas, which may include day, evening, weekend and Bank Holiday working and sleeping-in duties at night. (flexible)

 Help cover the work of the team, during absence, vacancies, of when a colleague is under pressure.

 Participate in the opportunities provided for training and development

 Provide First Aid services at the house/service when necessary and if trained to do so.

 Assist the individuals we support to present themselves as valued members of the local community.

 Create opportunities for social and leisure activities with service users.

Ensure that each individual is supported as far as possible to exercise their rights in the following areas: Privacy, choice, participation in decision making affecting their lives, expression of their own cultural identity, entitlement to service, feedback on the service they receive and the right to complain, protection of their property, maintaining links with family and friends.

 Liaise with other professionals on behalf of the individuals we support. (advocacy)

 Maintain contact with a wide range of outside professionals, eg. General practitioners, dentists, and therapists etc.

 Liaise with Day Service providers.

 Assist and participate in the wide range of administrative tasks necessary for the smooth running of the house/service.

 Participate in regular supervision to: receive support, increase knowledge, identify training needs, evaluate work performance, aid and assess professional development.

 Participate in an annual appraisal coordinated by your line manager.

 Attend staff development programmers, training courses, seminars and workshops as required.

 Promote equality of opportunity and a respect for diversity.

 Be an active member of the House/Service team.

 Help cover the work of the team, during absence, vacancies, of when a colleague is under pressure.

 Liaise and coordinate with other team members to provide a cohesive, high quality service.

 Share with other team member’s previous experience, skills and knowledge, which may be relevant to the team in providing its service.

 Be supportive, respectful and empathic to service users, colleagues and visitors.

 Undertake any duties consistent with the overall purpose of the post as directed by the Team leader, deputy Manage or Manager.

Key working:

 The keyworker will act as advocate, where appropriate, for their key tenant, ensuring that as an individual they receive the dignity, respect and rights that anyone else can expect.

Notes

 The post holder will refrain from smoking in any area of the Elegant Care Services (ECS) premises not designated as a smoking area.

 The post holder will abide by the Elegant Care Services (ECS) policy on alcohol consumption.

 The above job description may be adjusted according to the developing service needs in conjunction with the post holder
