Remuneration: Rate @ 50%

Description: Part-time Private dentist required for 3 days per week in a busy, PRIVATE practice near Newcastle-under-Lyme, Staffordshire
•    Wed - Fri - 9.am to 5.30pm
•    50% split on private and all labs
•    Fully computerised surgeries
•    Digital x-rays
•    Apex Locator & Rotary Endo
•    On-site dental lab with Cerec milling units
•    CT Scanner
•    Invisalign & Cfast treatments available in this practice
•    Full support team with Hygienist 
•    Free car parking available
•    Available Jan 24, start date negotiable

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number
