Contract Type : Permanent/ Part-time
Hours of Work : Approximately 18 - 21 hours per week

As an associate case manager, you will work with people who have suffered catastrophic injury to:

• Assess their needs, understand them pre-injury and identify their needs post-injury
• Put plans in place based on these needs for a successful rehabilitation with clear, manageable goals
• Implement the plans alongside the client (including the parent or carer of children and young people)
• Coordinate the relevant care and support whether that be accessing statutory services, therapy and rehabilitation, financial support as well as educational needs, vocational support, hobbies and lifestyle choices
• Monitor progress and respond to changing needs.

To Provide a flexible support service to an adult female with a spinal cord injury.
The support worker is required to work with the client assisting in any needs required.

The role will mainly require all aspects of personal and domestic care tasks to be undertaken and integrated with the community. You must be honest and reliable and have excellent interpersonal and interaction skills.

No experience is necessary as the most important factor is being able to get on with the client. On-the-job training will be undertaken & you must be able to work alone and use your own initiative.

Vacancy hours for the new position are as follows;

• Week 1 = 21 hours
• Monday 8am - 12pm
• Tuesday 9am - 12pm
• Thursday 9am - 12pm
• Friday 8am - 12pm & 2pm - 9pm

• Week 2 = 21 hours
• Friday 2pm - 9pm
• Saturday 2pm - 9pm
• Sunday 9am - 4pm

• Week 3 = 18 hours
• Monday 4pm - 9pm
• Tuesday 4pm - 9pm
• Friday 4pm - 9pm
• Saturday 9am - 12pm

There is only 1 day that there will be a split shift

• New Starter Probation period Rate of Pay
• Weekday - £11.50
• Weekend - £12.30
• Sleep-in - £11.44 ( both weekday & weekend )
• Bank holiday weekday £23.00
• Bank holiday weekend £24.60
• Bank holiday sleep-in £22.88 ( both weekday & weekend )
• Overtime weekday £15.23
• Overtime weekend £16.40

Due to the nature of this role, an Enhanced Disclosure from the Disclosure & Barring Service will be required.
