Job Title Advanced Nurse Practitioner (ANP) Reports to Clinically: GP and Nurse Partners Non-Clinically: Practice Manager Job Summary Working independently via telephone, through face-to-face consultations or home visits, you will be accountable for delivering safe, clinical decision-making and expert care to a high standard. Acting within your professional boundaries, your duties will include all (or part) of the patient journey including initial history taking; clinical assessment; diagnosis; treatment; prescribing and evaluation of their care. Key Responsibilities Work autonomously and be accountable for his/her own professional actions. Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans when working in a clinic or home visit environment. Through telephone clinical assessment, obtain a relevant and sequential history of the presenting complaint to determine a working diagnosis Formulate a management plan for the patient. And / or to use other sources of healthcare professionals (such as GP or Pharmacists) if required to support diagnosis or next course of action Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care Prescribe medication as required within your clinical competence. Recognise the need and initiate emergency treatment in order to save life or to safeguard the interests of the patient and refer to emergency services when appropriate Recognise any social needs, safeguarding concerns and act appropriately to safeguard the patient To ensure that patients are safely discharged from our care with appropriate instructions for after care and follow up Identify opportunities and to proactively encourage health promotion and health education to all patients To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence To take an active role within the cross-disciplinary, multi-skilled team to ensure service quality in line with corporate goals and strategies and provide mentorship and support to junior staff when required Take a person-centred approach to service delivery, based on clear understanding and experience of the modes for service delivery. To assist in the development of new models for service delivery which have a positive impact on the health economy Maintain and develop clinical competence and knowledge of care delivery within urgent care setting using both formal and informal training methods. To work within the professional Code of Conduct and other directives as defined by the NMC. To undertake appropriate personal and professional development to meet the requirements of NMC Registration. To participate in regular performance review with the Partners and / or Senior Management team To undertake further specialist training as dictated by service needs and the changing requirements of service provisions Clearly demonstrating at all times adherence to Northumberland Park Medical Group's core values Additional reasonable duties as directed by the business that are not explicitly stated in this job description
