• Social Work England Registration
• Extensive Adults Social Work Experience
• Experience of working with older people, people with disabilities, and long term health conditions and their carers.
• Knowledge of the Care Act and Mental Capacity Act

This local authority based between London and Kent is seeking an experienced Social Worker to be part of Integrated Triage Team.

You will responsible for working with service users and their carers, to promote and maintain their dignity, choice and independence. To assist as part of the multi disciplinary team in improving quality of life by, devising programmes to maximise independence and assist with accessing resources to provide practical help, where appropriate.

This team provides a multi-professional screening service for all referrals received by Adults Social Services using a 'see and sort' solution-focused model of intervention. It provides a Duty response for the Complex Care Teams and Community Assessment and Rehab Team.

As a Social Worker in the team you will have an opportunity to work in a creative and strength focused way. We are looking for Social Workers who embrace creative assessment and care planning working with informal support networks, understand community engagement and partnership working.
