Job description
Supporting clinicians with undertaking basic clinical activity such as history taking, conducting urinalysis, performing phlebotomy, taking blood pressure readings, administering vaccinations, checking a pulse, and possibly doing ECGs. (Training will be provided for any clinical activity that is required) Creating and maintaining personalised and patient centered care plans Delivering NHS Health checks in surgery and within the local community Tackling Health inequalities, promoting patient engagement and health promotion both within surgery and in and around the local community To seek out and develop opportunities, organise and deliver health checks and promotion within local community settings Arranging appropriate referrals, signposting patients, arranging tests and follow up appointments To deliver a range of back-office health care administration duties such as sorting, prioritising and signposting incoming clinical correspondence, extracting required information and applying medical codes to patient records Quality improvement auditing and reporting Attend, participate and support meetings In addition to the primary responsibilities above, the GP assistant may be requested to: Participate in local initiatives to enhance service delivery and patient care Support and participate in shared learning within the organisation Any other admin or clinical related work as provided by the PCN Executive team/Practice Managers
