Job description
PLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FURTHER DUTIES OF THE ROLE. To work within the Integrated Care Team, providing care to patients in their own homes / care home / other care settings providing assessment and treatment of patients in the community over a 8am-6pm period 7 days a week. To carry out planned and unplanned health (nursing) interventions as designated by the lead clinician, and will work both supervised and unsupervised in patients own homes, residential, nursing and other intermediate care facilities, reporting back patient progress and informing the lead clinician of any problems. The postholder will work flexibly across boundaries of care to support patients in their own homes based on identified need
