Job description
We are inviting applications for a permanent Grade 10 Advanced Practitioner to support the current Practice Manager's, Julie Ollier and Taffi Toole, within the Short-Term Services South Team. We are currently hybrid working from the office and at home. The successful applicant will report to and be supported by Julie Ollier and Taffi Toole.
The focus of the Advanced Practitioner is on ensuring that practice standards are maintained and that assessments are strengths-based and person-centred, whilst ensuring that interventions represent a proportionate response to presenting issues. The Advanced Practitioner will work closely with the Practice Manager and focus on quality assurance for practice related issues within the teams through supervision and informal support for staff. The Advanced Practitioner will give guidance to staff to support their decision-making and to help them tackle challenging conversations and conflict. The Advanced Practitioner would not be expected to deal with budgetary, staffing or team performance issues but is likely to hold a small, complex, caseload.
This is an exciting opportunity to develop leadership skills and undertake a different role. The successful candidate will have relevant, up-to-date experience of working Adults and their families, through an asset-based approach, which promotes health and well-being. The candidate must have experience in working collaboratively with other professionals. They also need to be able to work under pressure and support both the Manager and the workers within the transition team and a small but complex case load.

The Short-Term Services team works to Home First principles, and in close partnership with colleagues in acute and community health services, social care, and the voluntary sector. The local health and social care economy are working to embed and improve Home First working and with continued service development it is an exciting time to join the team. The team is community based with in-reach into hospital and community bed settings. The team is innovative, friendly, and supportive, and there are good opportunities for training and professional development within the job role.
What we are looking for:
The post holder will be based at Delamere House in Crewe, but hybrid working is available and includes working from home, there may be a necessity to travel to other bases. Cheshire East operates a 'flexible and mobile' working policy.
