Acute Liaison Nurse, Learning Disabilities - Western Sussex Hospitals NHS Foundation Trust (Royal Sussex County Hospital in Brighton) band 6 This is an exceptional opportunity for an ambitious, dynamic, and self-motivated learning disability nurse who can provide expert guidance in all aspects of care delivery for people with learning disabilities (PWLD), their families, carers and the wider stakeholder agencies and multi-professional teams that work within and in partnership with Western Sussex Hospitals NHS Foundation Trust. The post is funded by Sussex Partnership NHS Foundation Trust.
We are looking for applicants who will be visible on site and can balance detailed and rigorous clinical practice along with the ability to be visionary, flexible and self-motivated. The successful applicant will have a sound working knowledge of relevant legislative guidance including the Mental Capacity Act, the Equality Act along with working knowledge of contemporary drivers in Learning Disability practice. They will have a strong back-ground of working in learning disability service provision, and be able to evidence continued professional development
Main duties of the job Broadly, the role will include the following:
- To facilitate and co-ordinate care during, preparation, attendance, admission and discharge for People with Learning Disabilities (PWLD) in RSCH.
- To act as a resource to improve the experience of PWLD receiving services in RSCH
- Implementation of education and ongoing support of all health professionals within RSCH and other secondary and supporting services. To ensure good service delivery for PWLD.
- To update and disseminate relevant resources and accessible information
- To support review of the Learning Disabilities Policy
- To assist in monitoring the effectiveness of the Liaison Nurse Role and make recommendations for improvement
- Establish links and raise awareness with community agencies in order to implement integrated working in preparation for access to the acute setting
- Support appropriate and timely implementation of the Mental Capacity Act (MCA) and the Equality Act in line with due legal process
- Caseload manage individuals with chronic health conditions whilst in the acute setting and work jointly with provider and community LD teams in developing care plans to prevent unnecessary admission and re admissions.
- In line with findings from the Learning from Lives and Deaths (LeDeR) reports work with others to reduce the risk of premature and avoidable death in PWLD

Detailed job description and main responsibilities Main Duties: - Through specialist knowledge and experience, guide health care staff within RSCH to better understand the complex presentations associated with PWLD and to anticipate possible problems and further develop individualized care management plans for PWLD
- Collaborate with other health care professionals/relevant agencies within RSCH to promote planned and coordinated care
- Demonstrate a highly developed interpersonal approach with team members, patients, staff teams, families and supporting agencies where there are sensitive and contentious situations
- Active promotion of the liaison nurse role in RSCH, ensuring information is accessible to PWLD and their carers
- Assist PWLD, staff and carers pre-admission, at admission, during their hospital stay and at discharge to facilitate excellent care
- Facilitate an environment in which ethical issues are recognized, debated and acted upon.
- To meet regularly with identified Lead in RSCH.
- To actively promote equality and diversity issues for people with learning disabilities in RSCH
- To promote the use of patient held "health action plans" when clients access RSCH
- Adhere to the guidelines laid down by the NMC Code of Professional Conduct and standards on Records and Record Keepin
- To be involved in assessments of capacity to consent and multi- disciplinary best interest meetings for PWL
- To understand the national context which informs learning disability practice as it relates to the acute hospital setting e.g. findings from the learning from lives and deaths (LeDeR) reports and implications for practice in preventing avoidable and premature deaths of PWL
To be able to provide and receive highly complex information, in difficult and challenging situations, and be able to address and resolve barriers to understanding and acceptance of clinical and behavioural interventions where there may be different options. This will involve sharing relevant information to patients, staff teams, families and other supporting agencies
Person specification Qualifications Essential criteria - Current NMC Registration of 1st Level Nurse in Learning Disability
Knowledge/Experience Essential criteria - Sound level of evidence based clinical practice
