Job description


To work as part of a multi-disciplinary team and provide the medical management for the 21 patients in our rehabilitation hospital. All patients will be reviewed during your weekly ward round, medical management plans will be set and reviewed.

As Clinical Lead of HCT Inpatient Units, you will work with the ward staff to ensure consistency of service provision across the three inpatient units. You will play a key role in providing training, teaching and development opportunities for the junior medical staff and multidisciplinary team on the units.

You will be involved in working with the in-reach team to improve the quality of referrals into the units. You will also support the improvement in record keeping on SystmOne within the units and ensure a robust discharge process from our services. You will be involved in driving quality improvement and reviewing clinical incidents alongside the community beds leads, pharmacist and quality lead.

You will be expected to work as a senior clinician in the system and in this role, you will be expected to carry out regular weekly ward rounds with the team at Queen Victoria Memorial Hospital. You will also be expected to arrange and attend regular clinical meetings to discuss clinical cases, new developments in the service and enhance team working.

• Deliver high quality medical support to patients looked after by HCT
• Undertake full ward round of all patients each week
• To lead on the medical management and clinical safety of our inpatients
• Highlight any concerns regarding service delivery and patient safety
• Complete Medication reviews
• Provide clinical supervision to GP trainees
• Lead on complex decisions regarding End of Life / palliative care
• Effectively communicate with patients, and their families, to ensure they are aware of and involved in their medical management and plan.
• As opportunities arise teach ANP and ward nurses
• Act in line with the NHS constitution
• Act in line with HCT values
• Observe policies and procedures written by the trust
• Comply with GMC standards of Good Medical Practice and the NHS Constitution
• Undertake all required training and development to maintain skills/ accreditation
• Working with members of the multi-disciplinary team to facilitate safe and effective discharge for patients
