Job description

Salary: £10.92ph basic rate with enhanced rates for weekend & bank holidays
Even though this is a community-based role, you'll be paid hourly from the time your shift starts to the time your shift ends. In addition we pay £0.30p per mile towards your mileage.

Location: Chelmsford, including CM1 & CM2 areas

Are you passionate about providing support and assistance to individuals in the comfort of their own homes? We're seeking dedicated Care Practitioners to join our team, committed to ensuring the safety, dignity, and independence of our service users.

Role Summary:
As a Care Practitioner, your mission is to deliver personalized care plans, promoting a secure and respectful environment for service users. From personal care and household management to effective communication with service users and stakeholders, your role is vital in enhancing the quality of life for those in need.

Duties Include:

-Assisting with dressing, grooming, and personal hygiene
-Supporting daily activities and trips out
-Carefully managing toileting, continence, and pressure areas
-Collaborating with healthcare professionals on rehabilitation programs
-Providing sitting care services and healthcare reminders
-Preparing nutritious meals according to preferences
-Offering assistance with feeding and dietary care
-Performing domestic duties such as cleaning, bed-making, and laundering
-Working collaboratively with other care professionals and using manual handling equipment as needed

Responsibilities:

-Treat all service users and their families with dignity and respect
-Participate actively in reviews of individual care plans
-Maintain awareness of tasks that should not be undertaken as part of care duties
-Keep accurate and confidential records
-Foster positive communication with colleagues, families, and external organizations
-Explore opportunities to use assistive technology to enhance independence
-Report incidents, especially those related to safeguarding, to the Manager promptly
-Adhere to policies, procedures, and guidelines, and engage in continuous personal development as a valuable team member.
