Job description

Voluntary Centre Services is pleased to open recruitment for 2x Social Prescribing Link Workers to join our existing Social Prescribing Teams in the South Lincoln Healthcare Primary Care Network and Lincoln Central IPBT areas.

The Social Prescribing Service, developed alongside the Integrated Neighbourhood Working (INW) programme and Integrated Place Based Teams, provides a means of enabling primary care services to refer patients with social, emotional or practical needs to a range of local, non-clinical services.

Social Prescribing Link Workers are responsible for delivering a person-centred service, supporting people to improve their health and wellbeing by connecting them with services in their community. Link Workers are a vital community resource, responsible for patient engagement and developing effective working relationships with health care professionals and voluntary/community sector providers.

The vacancies are:

Lincoln Central IPBT (based from Lincoln) 0.7 FTE

Lincoln South (based from Sleaford) 0.6 FTE

This role involves regulated activity and requires an Enhanced level DBS check.

Annual Leave entitlement 29 days per year (pro rata)

Place of work to be Sleaford or Lincoln, dependent on role.
