Remuneration: £36 - £38 per hour    

Description: Part time Therapist required for 2-3 days per week in a busy, mixed INDEPENDENTLY OWNED practice near Swansea, South Wales.
•    Days Flexible
•    £36 - £38 per hour depending on experience and skillset.  Newly qualified considered
•    Hours, Mon-Weds 8.30am - 5.40pm, Thurs 8.30am - 4.40pm & Fri 8.30am - 4.10pm
•    Taking over an existing and well maintained list
•    Fully Computerised, SOE
•    Digital x-rays
•    Nurse available
•    70% Therapy 30% Hygiene
•    Experienced support team
•    Established since 2001
•    30-40 mins from Swansea, 30 mins to Carmarthern

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number
