Experienced Social Workers should be able to undertake person centred and strengths-based assessments either within the individual’s own home, hospital or Care Home, with the aim of improving client outcomes for the residents of Cheshire East. Additionally, this post will provide a wide range of learning and training opportunities and includes integrated working across the Health and Social Care sector. The Short-Term Services team works to Home First principles, and in close partnership with colleagues in acute and community health services, social care, and the voluntary sector. The local health and social care economy are working to embed and improve Home First working and with continued service development it is an exciting time to join the team. The team is community based with in-reach into hospital and community bed settings. The team is innovative, friendly, and supportive, and there are good opportunities for training and professional development within the job role
What we are looking for:
The post holder will be based between Macclesfield Hospital and Macclesfield Town Hall, but hybrid working is available and includes working from home, there may be a necessity to travel to other bases. Cheshire East operates a 'flexible and mobile' working policy.
