Job description
The post holder is an integral senior member of the All Age Continuing Care (AACC) Service and is committed to the appropriate assessment and commissioning of services to individuals who are registered with GPs within the Kent & Medway ICB area. The post holder will work closely with other senior colleagues in AACC to ensure that the policies and procedures of the National Framework are routinely integrated into health and social care work practices and that the ICB meets its statutory requirements. The post holder is responsible for the Neuro Rehab pathway and the caseload of patients funded within this pathway by the ICB in the North Kent and Medway areas.
