KEY RESPONSIBILITIES/OUTCOMES: Clinical Liaise with key stakeholders (as outlined above) to identify carers who may have support needs Compile a daily list of carers who have been identified by clinical staff as having additional support needs that are affecting the discharge of the patient Conduct a holistic needs assessment using a process as advised by clinical and community colleagues for each carer who comes to your attention Support staff to implement the Trust Carers Policy with and for carers, for example, authorising and giving out Carers Passports, accommodation for carers, access arrangements for end-of-life situations and advising on issues such as hospital parking. Be available to clinical colleagues and carers across the Hospital to advocate for their involvement in discharge planning, resolving issues and removing barriers to a positive patient and carer experience Work with colleagues across the Hospital, in the community and on your own initiative to support timely responses within 24/48hrs of referral that will remove barriers to discharge for the carer Advise on and implement individual Carer Support Plans that support the health and social needs of the carer, alongside the discharge planning for the patient who is the person they care for Advise carers about their legal rights, local rights in accordance with the Trust Carers Policy and advocate for carers involvement in planning for the discharge of the patient they care for Refer to other services internally as required, including adults and child safeguarding, Dementia Team, LD Team and others. Signpost carers to the available support networks available to help carers come terms with the emotional challenges of a new diagnosis or change in the health of a loved one Refer carers to our partners in the community to respond to the needs of the carer within 48hrs of referral using approved methods, e.g. online referral form Liaise with community partners to assist the Trust in the development of information resources both on and offline aimed at carers who use hospital services or anticipate using them in the future Act as a point of contact for community partners, including social care and local charities supporting carers to advise and link with other clinical professionals where they have a concern about the discharge of a patient in our care and its impact on the carer Act as a source of referral and approval for using the Winter Pressures Carers Emergency Fund, keeping accurate records of expenditure and receipts. Administrative Maintain accurate notes on CRS about actions taken to support the carer within the patients notes Maintain accurate records about own activity in order to monitor how your time is spent to inform the future development of the role Complete referral forms and write letters to third parties on behalf of carers as commensurate with the role. Team Role Work collaboratively with the Multi-Disciplinary Team to identify carers and develop individual support plans, as well as advocating for their needs. Work as an active member of the Transfer of Care Team, collaborating with colleagues at all levels to achieve your goals With your manager, set SMART objectives for the first 3 months and remaining 9 months of secondment
