My Client is currently seeking a highly skilled and motivated Clinical Pharmacist to join their dynamic team in Milton Keynes. The successful candidate will play a crucial role in delivering high-quality pharmaceutical care to their patients while collaborating with healthcare providers to optimize medication therapy outcomes.
Responsibilities:
Provide comprehensive pharmaceutical services, including medication review, therapeutic drug monitoring, and patient counseling.
Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans.
Conduct medication reconciliation to ensure accurate and safe transitions of care.
Participate in multidisciplinary rounds and contribute to clinical decision-making processes.
Monitor for adverse drug reactions and medication-related problems, intervening as necessary to mitigate risks.
Educate patients and caregivers on medication use, adherence, and potential side effects.
Stay abreast of developments in pharmacotherapy and contribute to continuous quality improvement initiatives.
Maintain accurate patient records and documentation in accordance with regulatory standards.
Requirements:
Bachelor of Pharmacy degree (Master of Pharmacy preferred) from an accredited institution.
Registration as a pharmacist with the General Pharmaceutical Council (GPhC).
Proven experience in a clinical pharmacy setting, with a strong understanding of pharmacotherapy principles.
Excellent communication, interpersonal, and problem-solving skills.
Ability to work effectively in a fast-paced environment and collaborate with diverse healthcare teams.
Commitment to ongoing professional development and quality improvement.
Familiarity with pharmacy software systems and electronic health records is advantageous.
