Job description

Search are currently looking for Band 6 Community Nurses/District Nurses to work within the NHS in the Hanley area. You will also help with the Virtual ward.

Ideally they would like someone full time. Monday-Friday, 08:00-20:00. If you can commit to 3-4 days each week let me know and I will see if the client will accept.

You will enable people to be cared for in their own community and remain as independent as possible, rather than having to go to hospital. The teams are supportive and friendly and truly make a difference to peoples lives and our passionate about patient care.

Role Summary

The post holder will work alongside the Virtual Ward Matrons and Nurse Practitioners to coordinate care. This will include the monitoring and management of Virtual Ward patients utilising both face to face home visits, and the use of digital monitoring systems (e.g. DOCOBO and COPD Predict). The post holder will be expected to use autonomous clinical assessment and decision making to manage and potentially escalate any deteriorating patients.

The role will include a significant clinical aspect, the post holder will be expected to be responsible for clinical assessment, management and evaluation of the Virtual Ward patient plan of care. This will include health assessment and formulation of medical management plans for acute patients that have been admitted to the ward from the CRIS team as hospital avoidance, as well as patients that have been discharged from hospital as supported early discharge.

Demonstrating safe and effective practice in line with the NMC Code of Professional Conduct and trust values and behaviours. The post holder will also be expected to perform regular clinical supervision with other members of the team, and will be involved in the appraisal process of a delegated number of staff.
