Remuneration: 45% Remuneration (5 days TAKE HOME circa 9k-10k per month)

Description: Full-time OR Part-time PRIVATE Dentist required for 2-5 days per week in a busy, mixed, well established PRIVATE Independently owned practice near Evesham, Worcestershire
•   45% Remuneration (5 days TAKE HOME circa 9k-10k per month)
•   Labs 50% split
•   Mon-Thurs 9am-6pm & Fri 9am-2pm
•   7 Fully computerised surgeries with R4 software
•   Treatment Coordinator
•   Clinical Freedom
•   Hygienist 
•   Digital X-rays
•   Intra oral cameras
•   Rotary endo
•   OPG
•   Performer number would be an advantage for a small kid's contract but not essential
•   40 minutes from Cheltenham

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number
