Job description
Be responsible for the analysis and adjustment of complex patient pathways and able to support and advise both internal and external clinical teams and patients. Ensure that every patient and health care professional that utilises the MFT smoking cessation service has an excellent experience of it Provide both verbal and appropriate written information to patients about their forthcoming appointments. Assist medical/Nursing staff to triage patients into the service and identify those patients suitable for other services support and clinical trials. Track patients throughout their journey, against the relevant patient pathways ensuring that patients are managed well within the key performance indicators, liaising with the relevant ward/department. Take appropriate action/escalate any delays with the relevant clinician and directorate management teams to prevent poor experience. Use judgement involving facts and situations when escalating issues to lead clinicians and Trust management. Feedback to relevant departments on assessment results, agreed treatment plans, occasional completion of further assessment. Access and manipulate several trust computer systems to track patients progress, patients case notes and the status of diagnostic tests. Work with the MFT CURE team attending team meetings, using video conferencing link where required, as a core member of the team. Act as a focal point for the MFT CURE service, liaising closely with various departments/other hospitals to co-ordinate the timely flow of all assessment results/information to ensure the patient can complete their journey in a timely fashion, being constantly kept in the loop about their various appointments. Collate all relevant information for the weekly team meeting and record data electronically for research and audit purposes. The post holder will require knowledge of many software programmes to create, maintain and manipulate various complex programmes, collecting and uploading data locally and nationally when required. Adhere to trust policies, communicating the patients diagnosis to the GP and informing referring hospitals of patients details both within 24 hours. The communication of patient details to a referring trust is currently a GMCCN target and is monitored by Trust Executive teams across the network. Attend any relevant Group/Business meetings, in the specialist area and other related meetings as and when required. Assist the team to maintain the service database to allow them to perform internal audit and research by ensuring initial information is entered onto the database and by setting up a system to obtain medical notes at relevant points in the patients journey. 16 Assist the Respiratory team to meet the measures set in the national targets. There is potential to provide cross cover for other admin staff as appropriate and assist in the induction and training of new staff. Undertake general office duties, including designing and implementing filing systems to ensure safe tracking of patient information through the CURE pathway.
