CLCH Hertfordshire Adult Community Services -CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

An opportunity has arisen for a Registered Nursing Associate (NA) or Assistant Practitioner (AP) to join our Adults Community Nursing Team based in Hertsmere.

Hi, my name is Hannah and I am the District Nurse Team Lead. I have worked in the Hertsmere team for several years, initially as a Band 5 nurse and now as a Band 7 Team Lead, having completed my District Nurse Specialist Qualification and getting lots of development support.

The community nursing team is part of an integrated locality team consisting of Community Nurses, District Nurses, Community Paramedics, Physiotherapists, Occupational Therapists, and Health Care Assistants. The Community Nursing team covers 8am-8pm, 7 days a week.

Our aim is to be the best we can, offering safe, patient-centred and evidence based care to those accessing our service.

Applicants must have the relevant NA or AP qualification already in place, and ideally will have had previous experience of working in the community. Key skills to demonstrate in your application are your clinical skills and experience, team work, communication skills, and willingness to learn. We are a home visiting service, so a driving license and access to a vehicle which is available for work use are essential requirements for the role.

The successful candidate will be enrolled into our 12 week induction programme, which will include local orientation, an individualised development review and training programme, working with the wider MDT, and opportunities to shadow key members of the team while you find your bearings and build your confidence in the role.

You will have regular input from the Professional Development Nurses and your Team Leader to support you in gaining the competencies relevant to your role. We also have a great track record of supporting Registered Nursing Associates to complete the Registered Nurse Degree Apprenticeship Programme.

We offer flexible shifts (8.30am - 4.30pm / 8am - 6pm) amd will consider applications from either full time or part time candidates. There is a requirement to contribute to our weekend rota, typically working three weekend days out of every four week roster cycle.

• Assessment and planning of delegated care
• Monitoring of long-term conditions and well-being of service users including escalation of interventions when necessary
• Assist with the implementation and monitoring of clinical care and monitor outcomes
• Administering of Medication in accordance with regulatory guidelines for Nursing Associates
• Undertake onward referrals to appropriate services and community based care support
