Remuneration: Circa £130,000+

Description: Full or Part-time Performer Dentist required for 2-5 days per week in a BEAUTIFUL, mixed INDEPENDENTLY OWNED practice near Bracknell, Berkshire.
•            Monday to Friday and some Saturday mornings - flexible working hours
•            EXCELLENT PRIVATE POTENTIAL
•            £13/£14 per UDA (the more UDAs the higher the rate)
•            50% split
•            PRACTICE OFFERS IMPLANTS & INVISALIGN
•            Full clinical freedom
•            8 Modern & Fully computerised Air Conditioned surgeries
•            iTero Scanners, OPG, Implant Motors
•            Digital x-rays, R4 software
•            Full support team including a Hygienist 
•            Existing, well maintained lists
•            Available now, negotiable start date

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number
