We are currently working with a Local NHS Trust who are seeking a Band 6 Community Nurse to join their team, based in Stoke on Trent.

What’s on offer:

• Working hours: The service operates 7 days a week, between 08:00-20:00, so shifts will vary between these hours.
• Contract length: 3-6 month initial contract with the view of extension
• Location: Stoke on Trent
• Rate of pay: Days: £26p/h, Nights/Saturdays: £30p/h, Sundays/Bank Holidays: £35p/h

About the role:

• Work alongside the Virtual Ward Matrons and Nurse Practitioners to oocrdinate care - This will include the monitoring and management of Virtual Ward patients utilising both face to face home visits, and the use of digital monitoring systems (e.g. DOCOBO and COPD Predict).
• Be responsible for clinical assessment, management and evaluation of the Virtual Ward patient plan of care - This will include health assessment and formulation of medical management plams for acute patients that have been admitted to the ward from the CRIS team as hospital avoidance, as well as patients that have been discharged from hospital as supported early discharge.

To be eligible for consideration, it is essential that you are a Registered Nurse with a valid NMC pin and have experience working within a community setting.
