Description: Part Time or Full Time Performer Dentist required 2-5 days per week for a mixed, Independent Practice in DISS, Norfolk
•    £15 per UDA
•    Principal happy to conver NHS to private
•    50% - 50% split EXCELLENT PRIVATE POTENTIAL
•    Days and Target Flexible
•    2 Fully Computerised surgeries
•    Digital x-rays, Rotary Endo, Intraoral Camera
•    Decon room
•    Busy, well established practice of 24 years
•    The Practice is working towards converting to Private so wishing to build Private List
•    Parking on Site
•    45 minutes drive from Norwich (24 miles) 50 minutes from Ipswich (25 miles)
•    Available now

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
