Salary: £36,576 - £38,480 (inclusive of 5% market supplement)

Hours per week: 37 hours

Hospital Discharge Service - Bridging Care, Ensuring Recovery

Our Locality Team (Discharge Service) are at the forefront of providing advice, assessment, and discharge planning for residents in Wiltshire. Collaborating

with healthcare professionals, we ensure smooth transitions for customers from hospital to home or care facilities. 

As a member of our team, your responsibilities will include conducting social work assessments, formulating support plans, and promoting independent living.

To excel in this role, registration with Social Work England is a must. Strong interpersonal skills and effective communication are essential. Your updated knowledge of relevant legislation, safeguarding vulnerable adults, and an in-depth understanding of Health and Social Care legislation and policy context are vital.

This position comes with a 5% market supplement payment. All market supplement payments are subject to review on an annual basis.
