Job description
the job holder will work under the supervision of a community clinical nurse specialist or paramedic to promote, plan and provide high standards of palliative care. to provide clinical expertise, to support and facilitate individualised patient centred care to patients and carers with a life limiting illness. to work in collaboration with the multidisciplinary team. to facilitate and support the implementation of evidence-based practice and contribute to the on-going development of the service. to participate in clinical and educational activities related to developing specialist palliative care in the community. to participate in a seven day per week operational service.
