Job description

To demonstrate developing clinical skills. To communicate complex condition related information from assessment to clients, carers, families, multidisciplinary team members and other professionals To work closely with clients, carers, families and multidisciplinary team members to ensure decision making relevant to client management To be responsible for assessment, differential diagnosis, formulation of treatment plans, writing assessment reports, providing appropriate intervention and evaluation of treatment outcomes for each individual client in respect of communication and/or feeding and swallowing difficulties with access to a senior colleague. To negotiate with carers, clients and others around individual case management To work with clients with dysphagia commensurate with experience and training. To access support from senior colleagues in respect of communication and/or feeding and swallowing difficulties. To be able to identify areas for own personal/professional development evidenced by personal development plan/professional portfolio developed within the appraisal and KSF framework. To comply with the professional guidelines of the RCSLT and HCPC. To be aware of and adhere to the Speech and Language Therapy Service Plan. To adhere to local clinical guidelines To contribute to discussions pertaining to proposed service delivery/policy development. To explain the role of the Speech and Language Therapist To maintain up to date and accurate case notes in line with professional guidelines and Trust policies. To work independently with access to supervision as per departmental, Trust and RCSLT guidelines. To be accountable for own professional actions To recognise own professional boundaries and to seek advice and support when necessary. To be aware of and adhere to current legislative and employer procedures relating to the post. To accept clinical support and advice from designated mentor/senior clinician To manage a caseload independently.
