Right now, we - re looking for anOccupational Therapist with community experience to join the Primary HealthPartnership, Rapid Response team. Thisservice provides a multidisciplinary approach to patient care and isfocused on, hospital avoidance, early supportive discharge (from hospital) andnon-acute rehabilitation in a homecare setting

The OccupationalTherapist(s) in this team work collaboratively with the Emergency Department,General Practice, Residential Aged Care and are part of a team that includes,Registered Nurses, Physiotherapists and the Health Care Assistants work underthe delegation of the Healthcare professionals.

This role will suitsomeone who is able to work with a wide group of healthcare professionals, havethe ability to practice independently and use prior expertise to ensure patientand client centered care enabling them to maximize independence.

To be eligible for this role, NewZealand Registration and a current Annual Practising Certificate is requiredalong with a current full driver license.
